Ny Health Care - Innovative NY Health CareAs expected New York is a leader in providing health care programs to its residents, in fact NY Health Care has so many programs that it is virtually impossible to cover them in this article. Therefore, we will discuss some of the most important targeting the aging and people with disabilities. First is the Steps to a HealthierNY program that helps New Yorkers of all ages live longer lives by adopting healthy habits to prevent or lessen the effects of diabetes, asthma, and obesity. Currently four counties participate in this program and they are Broome, Chautauqua, Jefferson and Rockland. With each of these counties providing activities to help residents exercise more, eat better foods and avoid tobacco use. Next is the Benefits Counseling program that addresses two of the prime concerns preventing those with disabilities that are receiving SSI from working. The first is the concern that they will loose benefits, and the second is the impact returning to work would have on rental payments. Thereby, addressing these concerns, "Benefits Planning and Outreach" agencies in New York State offers counseling to adult care facility residents who receive Supplemental Security Income (SSI) and/or Social Security Disability Insurance (SSDI) and who are interested in vocational and employment programs. The MTA Reduced-Fare MetroCard Program — New York City and Long Island residents who are either 65 years of older, or who have a physical or mental disability, are eligible for Reduced-Fare MetroCard discounts. To find out the information on this program and to get help filling out the application for it, New York City Transit MetroCard Mobile Sales Outreach staff is available throughout New York.
